How it works
Fillout is a form builder for collecting information and running intake processes. Add an existing, published form to your Zite app and tell Zite where to show it. Respondents use the form you select without connecting a Fillout account. To move a form into a Zite workspace, see Move a form into a Zite workspace. If you built the form in Fillout, the same steps apply. With this integration, you can:- Show a published Fillout form inside your app
- Collect responses using the questions and rules set up in Fillout
- Include file uploads or signatures configured on the form
- Add an existing booking or payment form
- Show a confirmation or next step after submission
How to integrate Fillout into your app

Connect to Fillout
Publish your form in Fillout, then click + in Zite’s chat box, select Integrations, and choose Fillout. In Select Forms, choose the forms to use and save. This picker uses forms available in your account.

Describe your use case
Tell Zite where the form should appear and what happens after submission. For example, “Add our Customer feedback form to the support page and show a thank-you message when it is submitted.”

Test and publish
Submit a sample response and check it in Fillout’s results page. If responses should appear in a Zite table, check that destination too.Click Publish in the upper right corner when the workflow works as expected.
Common use cases
With Fillout, you can create apps like:- Customer Feedback: Collect feedback from a support portal.
- Application Intake: Gather answers and supporting documents.
- Client Booking: Add an existing scheduling form to a client portal.
- Approval Intake: Collect requests before your team reviews them.
Important notes
- Published forms: Only published forms appear in the picker. If one is missing, check that it is published and available in your account.
- Form setup: Manage questions, conditional rules, payments, and booking settings in Fillout. Embedding the form does not create those settings for you.
- Saving to a database: Embedding a form does not automatically put every response in your app’s database. Configure the form’s destination and check any conditions that control when records are saved.
- Workspace access: A form may save data to another workspace. Your app cannot read that workspace’s database just because the form is embedded.
- Plans: Paid form features depend on your Fillout plan; a paid Zite plan does not include paid Fillout features.
